Minimum Qualifications

All applicants must meet the education and/or experience requirements as stated on this exam bulletin to be accepted to the examination.

Unless otherwise stated in the Minimum Qualifications, qualifying experience may be combined on a proportionate basis if the requirements stated below include more than one pattern and are distinguished as "Either" 1, "or" 2, "or" 3, etc. For example, applicants possessing qualifying experience amounting to 50% of the required time of Pattern 1 and additional experience amounting to 50% of the required time of Pattern 2 may be accepted to an examination as meeting 100% of the overall experience requirement.

Education Requirements: Equivalent to completion of the twelfth grade. (Additional qualifying experience may be substituted for the required education on a year-for-year basis.)

Two years of experience performing the duties of a Health Record Technician II (Specialist) or Health Record Technician II (Supervisor).

Position Description

00101873 - HEALTH RECORD TECHNICIAN III

This is the second supervisory level in this series. As a supervisor, under general direction, plans, organizes and assigns work areas to record coders and abstractors; resolves difficult technical abstracting and coding problems; analyzes quality control activities; acts as a consultant to records personnel in hospitals, laboratories, clinics and registries; conducts training activities and workshops; gives lectures; and consults with and advises superiors on operational matters.

Examination Information

Applicants must submit their exam application AND complete the exam during the same cut-off period.  For example, if you submit your exam application on July 8th, 2024 (which is after the July 5th, 2024 cut-off) you must complete the exam by July 19th, 2024.  Filing Cut-Off Dates are listed at the beginning of the exam bulletin.

In addition to evaluating the applicant's relative knowledge, skill and ability as demonstrated by quality and breadth of education/experience, emphasis in each test component will be on measuring competitively, relative to job demands, each applicant's:

Knowledge of: Basic medical terminology, human terminology and human anatomy; health record systems and methodology used by health facilities; classification of morbidity and mortality information for statistical purposes.

Ability to: Understand and conform to specific basic principles and rules of health data abstracting and coding; meet and deal tactfully with the public; communicate effectively; and benefit from academic and in-service training and job experience.

Eligible List Information

A Servicewide Open eligible list for the HEALTH RECORD TECHNICIAN III Classification will be established for:

The names of successful competitors will be merged onto the eligible list(s) in order of final score regardless of test date. Eligibility expires 12 months after it is established. Applicants must then retake the examination to reestablish eligibility.

Resulting Eligibility List(s) will be used to fill vacancies throughout the State of California.

Veterans' Preference will be granted for this examination. Veterans' Preference is an assistance program for veterans, veteran widows and widowers, and spouses of 100% disabled veterans who seek employment with the state. Veterans' Preference is also extended to currently serving military members nearing their retirement or separation date.

You are not eligible for Veteran's Preference if you are:

  • A veteran who has been dishonorably discharged or released.
  • A veteran, widow or widower of a veteran, or spouse of a 100 percent disabled veteran who currently has permanent civil service status. (Permanent civil service status is the status of an employee who is lawfully retained in his/her position after completion of the applicable probationary period. This does not include limited term).

Health Record Technician III Training and Experience Preview

Instructions: Rate your knowledge and experience performing specific job-related tasks.

 

  1. Oversee work areas of record personnel for the purpose of health record processing functions by monitoring, planning, and organizing activities and tasks to meet operational needs in accordance with applicable licensing and accreditation standards.
  2. Supervise health information management staff (e.g., supervisors, specialists, technicians, etc.) utilizing effective management principles to ensure efficiency and compliance.
  3. Participate in facility committees by representing the health information management department to provide subject matter expertise, collaborate, and meet operational needs of the facility.
  4. Train staff on health record issues and processes by developing educational materials and conducting instructional programs/activities to adhere to departmental policies and procedures, and licensing and accreditation standards.
  5. Compile written reports and/or graphic charts by gathering, tabulating and analyzing health record data to provide pertinent information to management as required.
  6. Train health information management staff in the areas of continuous quality improvement, and health record processing and operations to ensure adequate understanding of expectations in accordance with applicable licensing and accreditation standards.
  7. Oversee billing processes and practices to ensure quality and timeliness of data collection in accordance with reimbursement standards, and departmental policies and procedures.
  8. Oversee and/or monitor quality control activities for assigned sections by analyzing and implementing performance improvement audits to ensure functions of the department adhere to regulatory requirements.
  9. Alert the privacy officer through written or verbal communication concerning problematic privacy/confidentiality requests or complaints of reported/suspected violations, etc. in accordance with applicable State and Federal law.
  10. Maintain a variety of health record indexes or storage and retrieval systems to collect, classify, store, or analyze information under the supervision of the director of health information management.
  11. Oversee the compiling, abstracting, and coding of patient data using standard classification systems to ensure compliance with reimbursement requirements.
  12. Provide guidance as a subject matter expert in the area of health record processing functions by consulting and advising staff to ensure compliance with applicable regulatory requirements, and licensing and accreditation standards.
  13. Conduct meetings, such as quality improvement, to discuss program operations, to educate staff, and to ensure compliance.
  14. Resolve conflicting, missing, or unclear codes or diagnoses by communicating with providers to ensure accuracy and to optimize reimbursements.

  1. Knowledge of human anatomy, physiology, and medical terminology in order to review and convert medical data from census, audits, treatments etc. for statistical reporting.
  2. Knowledge of health record systems and methodology used by health facilities in order to oversee health information management activities and staff.
  3. Knowledge of morbidity and mortality classification systems (e.g., ICD-10-CM, CPT, DSM 5, etc.) to ensure accurate coding.
  4. Knowledge of a supervisor’s role in the equal employment opportunity program in hiring and employee development to maintain a work environment that is free of discrimination and harassment.
  5. Knowledge of administrative/clerical procedures and systems (e.g., basic office equipment, computer software programs, word processing, file management, etc.) in order to assist and supervise staff.
  6. Knowledge of supervisory principles, practices, and techniques to plan, oversee, and direct the work activities of subordinate staff.
  7. Knowledge of diagnostic techniques and modes of therapy in order to interpret detailed medical records for chart completeness and accurate statistical reporting.
  8. Ability to apply principles and rules of health data abstracting and coding to ensure compliance with licensing and accreditation standards.
  9. Ability to establish and maintain professional relationships internally and externally to ensure efficient communication.
  10. Ability to independently plan, organize, train, and direct the activities of health record technicians in order to adequately transfer skills and ensure compliance.

If you meet the requirements stated on this examination bulletin, you may take this examination, which is competitive. Possession of the entrance requirements does not assure a place on the eligible list. Your performance in the examination described in this bulletin will be rated against a predetermined job-related rating, and all applicants who pass will be ranked according to their scores.

Department of State Hospitals reserves the right to revise the examination plan to better meet the needs of the service, if the circumstances under which this examination was planned change. Such revision will be in accordance with civil service laws and rules and all applicants will be notified.

General Qualifications: Applicants must possess essential personal qualifications including integrity, initiative, dependability, good judgment, the ability to work cooperatively with others, and a state of health consistent with the ability to perform the assigned duties of the class. A medical examination may be required. In open examinations, investigation may be made of employment records and personal history and fingerprinting may be required.

Eligible Lists: Eligible lists established by competitive examination, regardless of date, must be used in the following order: 1) sub-divisional promotional, 2) departmental promotional, 3) multi-departmental promotional, 4) service-wide promotional, 5) departmental open, 6) open. When there are two lists of the same kind, the older must be used first. Eligible lists will expire in one to four years unless otherwise stated on the bulletin.

High School Equivalence: Equivalence to completion of the 12th grade may be demonstrated in any one of the following ways: 1) passing the General Education Development (GED) Test; 2) completion of 12 semester units of college-level work; 3) certification from the State Department of Education, a local school board, or high school authorities that the competitor is considered to have education equivalent to graduation from high school; or 4) for clerical and accounting classes, substitution of business college work in place of high school on a year-for-year basis.

Accepted applicants must provide proof of identification at each in-person test session. Acceptable identification consists of a photo identification card or two forms of signed identification.
