The Statement of Qualifications (SOQ) will be used to pre-screen applicants for the C.E.A. position within the Center for Healthy Communities. The SOQ is a narrative discussion of the candidate's experience which qualifies them for this position. The SOQ must be no more than four pages in length, using 12-point Arial font. Candidates are also required to include a resume with their submission; however, please note resumes will not be accepted in lieu of the SOQ. Answer each numbered item separately, indicating the corresponding item number for each response.
- The Substance and Addiction Prevention Branch values equity, diversity, inclusion, accessibility, and belonging (EDIAB). Provide specific examples of actions you have taken to foster EDIAB in the workplace. Include examples of how you incorporated health equity into program implementation and policy formation.
- Describe your education, experience, and knowledge of health and/or behavioral health promotion and prevention strategies as related to the duty statement.
- Describe your leadership style, skills, and ability, including specific examples of your experience executing on a vision; establishing objectives and key results; motivating and coaching employees; and aligning disparate programs.
- Describe your experience and approach to building and sustaining working relationships with internal and external partners – including community groups, local health and behavioral health departments, state agencies, and other entities.
- Describe your experience leading a significant change initiative within an organization or program. Include how you determined the effective course of action, what strategies you used, and the outcome of your action.

Qualifications requirements does not assure a place on the eligible list. Your performance in the examination will be compared with
the performance of others who take this examination, and all successful candidates will be ranked according to their scores.
A minimum rating of 70% must be attained in the Examination to obtain list eligibility for this position. All candidates will receive
written notification of their examination results.
The results of this examination will only be used to fill this position. Applications will be retained for twelve months.
