DSH-Napa is seeking a highly skilled and detail-oriented MDS Coordinator RN to lead the Minimum Data Set (MDS) assessment process for our Skilled Nursing Facility. This position plays a central role in ensuring regulatory compliance with CMS requirements, driving quality outcomes, and supporting the Resident Assessment Instrument (RAI) process. The MDS Coordinator works collaboratively with interdisciplinary team members, nursing staff, physicians, and ancillary departments to produce accurate, timely, and clinically sound assessments.
Key Responsibilities
• Coordinate, complete, and transmit MDS assessments in accordance with CMS RAI Manual requirements and established timeframes
• Develop and maintain accurate care plans in collaboration with the interdisciplinary treatment team
• Monitor compliance with OBRA assessment schedules and ensure timely electronic submission to CMS
• Lead or support QAPI initiatives related to clinical quality measures, hospitalization reduction, and documentation accuracy
• Serve as the facility's primary resource for MDS coding questions and RAI process guidance
• Collaborate with disciplines across the hospital to ensure comprehensive and accurate assessment data
• Analyze facility quality measure data and work with leadership to implement performance improvement strategies
• Prepare for and participate in CDPH and TJC surveys; respond to survey findings related to MDS and care planning
• Provide education and training to nursing and clinical staff on MDS documentation requirements
