The Integrated Systems of Care Division (ISCD) within the California Department of Health Care Services (DHCS) is hiring. We are seeking a proven leader with a passion for addressing priority health need in the community. The Care Coordination Section is looking for a Health Program Manager I who is motivated and is looking to make a difference in the lives of Californian’s. Health Program Manager (HPM) I will be responsible for the supervision of Unit B. The HPM I will coordinate and/or personally perform difficult decisions regarding nursing/medical eligibility, coordinate medical care needs, and manage enrollment into the Special Populations (SP) Programs including the California Children’s Services (CCS) program, the CCS Medical Therapy Program, the Genetically Handicapped Persons Program and Home and Community Based Services (HCBS) programs which includes the In Home Supportive Services, Waiver Personal Care Services, Alternative Living Waiver, Aids Waiver, and the Developmental Disability Services Waiver. The HPM I leads the unit team to conduct, review, validate, and document eligibility of referrals to the SP and HCBS programs, including evaluation of level of care determinations, person-centered service plans, and the integration of long-term services and supports for members with complex medical needs into the community. Performs other duties as necessary to ensure members are provided specialized health services; including occupational and physical therapy services, and medical equipment and supplies as necessary to maintain their health and well-being.

Effective July 1, 2020, in response to the economic crisis caused by the COVID-19 pandemic, the State of California implemented the Personal Leave Program 2020 (PLP 2020).  As a result, full time DHCS positions are subject to a 9.23% reduction in pay, in exchange for 16 hours PLP 2020 leave credits.  Part time and intermittent position compensation will be on a pro-rated basis.

This position is located in midtown Sacramento located in a multi-story building with plenty of access public transportation/parking and close to several excellent restaurants.
